I am an in network provider for the following Behavioral Health Insurance plans:

If you are covered by a different insurance plan and want to submit a claim for an out of network provider, I will provide you with a bill at the end of the month.

PAYMENT:

The full amount of the session fee is due at the end of the session.  I accept cash or check payments.
